Part 1:
Please base this reflection on the family you are studying. Build the reflection from the windshield document and reflection from week 2.
Write a minimum of 700 words reflection of the community assessment in which you address the following:
• Explain the role of community and public health nursing and community partnerships as they apply to the participating family’s community.
• Address the influence of cultural diversity and vulnerable populations as they apply to the participating family’s community.
• Explore the epidemiological influences within the participating family’s community.
Select one Healthy People 2020 Leading Health Indicator (LHI) that relates to your participating family’s community.

• Develop three community or public nursing diagnoses that relate to the participating family’s community based on the Community Assessment and Windshield Survey.
• Identify two potential nursing interventions related to each diagnosis that could have a positive effect on the selected LHI.
• Describe a community health partnership that could positively influence the selected LHI.
Note. Remove all personally identifying information such as the family members’ names. Refer to family members by initials only.
Format your reflection consistent with APA guidelines. Provide a minimum of 3 peer reviewed journal references that are cited in text as well.
Part 2:
Assess the health services available in your participating family’s community.
Examine 4–6 community and public health agencies such as the following:
Local health department, medical center, clinic, or hospital
Community health agencies
Public and private social service agencies
Schools
Conduct phone calls, interviews, and Internet research to evaluate the services provided by these agencies. Answer the following for each public health agency:
• Any community partnerships, coalitions, or planning groups
• The main health services and resources provided by each agency
• Each agency’s goals for achieving a healthy community
• How the agency addresses primary, secondary, and tertiary prevention
• How each agency meets the health needs of vulnerable populations within the community
• To what degree each agency reflects local cultural diversity
• Any gaps in services, funding, or staff
• How effective each agency is at meeting its goals
• How the agencies interact
• How your participating family might use the services of these agencies
