bowel movement in 3 days; or diarrhea, Drank less, Weight change, Agitated or nervous more than usual, Tired, weak,
confused, or drowsy, Change in skin color or condition, and Help with walking, transferring, toileting more than usual.
Overtime this practice had not been utilized since staff became complacent. Since implementation it was presented in a
piece of paper and CNAs or interdisciplinary members were supposed to provide them to the nurse as a part of the change
of status acknowledgement. As we all have learned, paper can get easily loss or toss and that is the issue of this type of
practice. According to the INTERACT (2016) organization “communication about changes in the status of residents in
skilled nursing facilities. The goal of this tool is to improve care and reduce the frequency of potentially avoidable transfers
